CVS Caremark Reverses Zepbound Exclusion, Adds Oral GLP-1 Foundayo to Commercial Formularies

CVS Caremark, the largest pharmacy benefit manager in the United States, is reversing a controversial decision. The company cut off coverage for one of the country’s most prescribed obesity medications last year. Now, it is bringing that coverage back.

Starting October 1, Zepbound will return to CVS Caremark’s standard commercial formularies as a preferred option. Additionally, the company will begin covering Foundayo, Lilly’s newly approved oral GLP-1 pill, starting June 1. Both changes apply to employers who choose to include these drugs in their plans.

The move ends more than a year of controversy. CVS Caremark’s original removal of Zepbound forced thousands of patients to switch treatments or pay steep out-of-pocket costs. It also triggered a class-action lawsuit filed in September, which remains active. As recently as May 15, 2026, plaintiffs argued that dropping coverage without adequate notice violated continuity-of-care provisions in their health plans.

Under the new arrangement, GLP-1 drugs from both Eli Lilly and Novo Nordisk will hold co-preferred status on CVS Caremark’s standard commercial formulary. In practical terms, that gives patients and physicians a real choice between the two leading injectable weight management treatments. Previously, coverage restrictions effectively steered patients toward one option.

CVS Caremark’s standard formulary reaches roughly 25 to 30 million people. However, formulary inclusion does not guarantee coverage, since plan sponsors retain the final say on whether to include GLP-1 therapies for weight loss. Nevertheless, CVS Health projects that the restructured formulary will generate savings of 10 to 15 percent across the weight-management category.

Notably, the other two major pharmacy benefit managers, Express Scripts and Optum Rx, have already been covering Zepbound. Consequently, CVS Caremark’s reversal now brings all three dominant PBMs into alignment on Lilly’s flagship injectable. That represents a significant shift in how the drug sits within commercial insurance benefit design.

Alongside the Zepbound reinstatement, CVS Caremark’s decision to cover Foundayo introduces a genuinely new option. The FDA approved Foundayo for chronic weight management on April 1, 2026. It contains orforglipron, a once-daily small-molecule GLP-1 receptor agonist. The drug is indicated for adults with obesity or for overweight adults with weight-related health conditions, used alongside a reduced-calorie diet and increased physical activity.

What sets Foundayo apart is its dosing flexibility. Unlike peptide-based oral GLP-1 agents, it carries no restrictions on food, water, or time of administration. In clinical practice, that flexibility may improve long-term medication adherence. Furthermore, clinical trials showed that the highest evaluated doses produced an average weight loss of around 27 pounds over 72 weeks.

Affordability, however, remains the bigger challenge across this drug class. Patients paying out of pocket for injectable GLP-1 therapies currently face monthly costs of $299 to $449, depending on dose. Oral options like Foundayo start lower, at $149 per month. For many patients, access still depends entirely on what their employer-sponsored plan elects to cover.

On the Medicare side, the Trump administration confirmed that both Zepbound and Wegovy will be available to Medicare patients for $50 per month starting July 1. Together, these developments suggest the coverage landscape for obesity pharmacotherapy is shifting in a meaningful direction.

Moreover, CVS Caremark’s decision arrives amid continued regulatory scrutiny of compounded GLP-1 formulations, which further reinforces the role of FDA-approved branded therapies within commercial benefit structures. For patients who lost access to Zepbound last year, the October 1 effective date marks a long-awaited change.
